AMATEUR ATHLETIC MINOR WAIVER & RELEASE OF LIABILITY: By checking the box & initialing box - you agree the Golden Pines Football Academy LLC will provide secondary insurance for each registrant. Primary coverage will be through each registrants own insurance plan. As the parent or guardian of the registered child, I hereby give my approval to his participation in the Golden Pines Football Academy Summer Football Camp. In case of injury to my child, I agree to waive all claims resulting from, or in connection with, the activities in which my child is a participant. I hereby release, absolve, indemnify and hold harmless the Golden Pines Football Academy LLC, its directors, its coaches, Seneca HS, the LRHSD, its members, officers, organizers, sponsors and supervisors and Tabernacle Twp, its members, officers, organizers, sponsors and supervisors. By checking the box & initialing you have read the above Waiver & Release, understand that we have given up substantial rights by checking & initialing box voluntarily.
